The one-time registration fee of only $50.00 is non-refundable, (completely tax deductible) and it entitles new members to permanent registration with no renewal required. You will have a lifelong membership with us. You will not need to register again in the future, but we do encourage all members to update their information with us if it changes…especially if you have found your family!

Once you have downloaded the form, please do the following:

1. Fill out with dark ink, and type or print clearly. You can also download the form and complete it with any Adobe Acrobat program.

  • Answer as many questions as you can.
  • Do not abbreviate.
  • Use a separate form for each child if a birthparent in search of more than one relinquished child.

2. Please provide to us all of the information you currently have. Please do not withhold any information as it makes it more difficult for us to assist you. Please indicate what you have already done in your search so we won’t have to repeat those steps. Also include:

  • Legible non-returnable copies of any background, non-identifying information received from the agency that handled the adoption, or the state in which the adoption was finalized. Do not send originals, as copies will not be returned to you. Please do not send original copies. You may submit non-returnable documents.
  • Use a separate sheet of paper to include any additional information you have which may not have been asked.
  • Please advise us if you are working with any other search/reunion groups, or search angels.

3. Adoptees applying for membership, please include a clear copy of your adoption papers (petition to adopt and final adoption decree), if you have them. Birth parents: If you have any nonidentifying information on the adoptive family, please include copies of that information as well.

4. Be sure to sign and date the second page. Your application will be returned if not signed and dated.

5. If you use a Post Office Box, please also provide an actual street address for our files.

6. Mail the completed form to the address below. (Do not use any special delivery service, or use too large an envelope that it won’t fit into the box).

7. The registration fee is $50. We accept PayPal. Please make sure to choose Registration as your payment and also make sure your name and your DOB is included somewhere on your PayPal submission so we may attribute your registration form to your online payment.  If you prefer, you may also mail a check in with your registration form.
